WORMS Dissertation Directory : Submission Form

OR/MS persons who completed their PhD theses recently (>1989) are encouraged to fill-in and submit the following form so that their dissertations will be listed in the WORMS Dissertation Directory. The process consists of the following steps:

  • Step 1: Fill in the form below and submit it using the Submit button at the bottom of the page.

  • Step 2: Read the Reply Page that will be generated automatically in response to your submission to make sure that the information you provided is correct. If corrections are required, make them and submit the form again.

  • Step 3: WORMS will process your form and will notify you when your record is added to the directory.

  • Step 4: Read your entry in the WORMS Dissertation Directory and let us know if changes are required.

  • Step 5: Inform us on changes to your e-mail address and/or WWW home page.

Remark: Most of the input fields are mundatory and must be filled-in. Optional fields are marked as such.
